Uses a collaborative process to ensure that assigned patients are managed appropriately and expeditiously through the health care continuum. Assesses, plans, intervenes, monitors, evaluates and coordinates the options and services required to meet the patient's health needs while promoting quality, cost effective outcomes.
Three years clinical experience; home care or home care coordination, utilization management, case management or discharge planning experience. Current NJ RN license and valid driver's license. Managed care experience preferred.
